Impact of shoulder pain on upper limb function: An observational study in post-stroke

dc.description.abstractShoulder pain in the hemiplegic upper limb is a frequent complication after stroke, with negative effects on functionality and quality of life. To analyze the impact of shoulder pain on upper limb functionality in post-stroke individuals, considering pain, motor, and functional parameters. An observational cross-sectional study was conducted in a rehabilitation service of a hospital in northern Portugal. Twenty-nine individuals with post-stroke shoulder pain participated. Pain severity and interference were assessed using the Brief Pain Inventory. Muscle tone was measured with the Modified Ashworth Scale, motor performance with the Fugl-Meyer Assessment, functional independence with the Functional Independence Measure, grip strength with a dynamometer, and manual dexterity with the Jebsen-Taylor Hand Function Test and the Box and Block Test. The mean pain severity score was 4,82 (standard deviation 1,72). Pain severity correlated strongly with pain interference, but not with motor or functional outcomes. Motor performance correlated positively with functional independence and grip strength, and negatively with muscle tone. Participants more than two years post-stroke showed poorer motor and functional results. Stroke type influenced pain interference, with higher values in ischemic patients. No sex differences were observed. Motor impairment and increased muscle tone were the main determinants of functional independence. Rehabilitation should prioritize motor recovery and tone control, while pain management may provide complementary benefits.eng
dc.description.abstractA dor no ombro no membro superior hemiplégico é uma complicação frequente após o acidente vascular encefálico (AVE), com impacto negativo na funcionalidade e na qualidade de vida. Analisar o impacto da dor no ombro na funcionalidade do membro superior em indivíduos pós-AVE, considerando parâmetros de dor, função motora e independência funcional. Estudo observacional e transversal realizado num serviço de reabilitação de um hospital do norte de Portugal. Participaram 29 indivíduos com dor no ombro após AVE Avaliaram-se a severidade e a interferência da dor através do Inventário Resumido da Dor, o tónus muscular com a Escala Modificada de Ashworth, a função motora com a Escala de Avaliação Fugl-Meyer, a independência funcional com Medida de Independência Funcional, a força de preensão com dinamómetro e a destreza manual Teste de Função Manual de Jebsen-Taylor e Teste de Box and Block. A severidade média da dor foi de 4,82 (DP = 1,72). A severidade correlacionou-se fortemente com a interferência, mas não com resultados motores ou funcionais. A função motora correlacionou-se positivamente com a independência funcional e força de preensão, e negativamente com o tónus muscular. Participantes com mais de dois anos após o evento apresentaram piores resultados. O tipo de AVE influenciou a dor, com maior interferência nos casos isquémicos. Não se observaram diferenças entre sexos. O défice motor e o aumento do tónus muscular foram os principais determinantes da independência funcional. A reabilitação deve priorizar a recuperação motora e o controlo do tónus, sendo o tratamento da dor uma estratégia complementar.por
