Santos, Paula Clara; Abreu, Sandra; Moreira, Carla; Lopes, Diana; Santos, Rute; Alves, Odete; Silva, Pedro; Montenegro, Nuno; Mota, JorgeThe aims of the this prospective study were to analyze physical activity (PA) engagement during the first and second trimesters, considering the different guidelines published on PA, to document the individual characteristics associated with the accomplishment of these guidelines and to examine pregnant women’s perceived barriers to leisure PA, using a socioecological framework. A sample of 133 pregnant women in two stages – at 10–12 weeks’ gestation (T1) and 20– 22 weeks’ gestation (T2) – were evaluated. PA was assessed by accelerometry during the T1 and T2 evaluation stages. Socio-demographic characteristics, lifestyle factors and barriers to leisure PA were assessed via questionnaire. A large proportion of women (ranging from 32% to 96%) did not reach the levels of PA recommended by the guidelines. There were no significant differences between T1 and T2 with regard to compliance with PA recommendations. A decrease in PA levels from T1 to T2 was noted for all recommendations. No associations were found between participants’ characteristics and adherence to the recommendations in T1 and T2. No significant differences were found in barriers to leisure PA between T1 and T2. The most commonly reported barriers to leisure PA were intrapersonal, not health related. Our results indicate that there were no differences between trimesters regarding compliance of PA recommendations, and perceived barriers were similar in both trimesters.
- Parâmetros do exercício na lombalgia gestacional: revisão sistemáticaPublication . Santos, P; Jerónimo, Ana Catarina da Silva; Alves, Odete; Mesquita, Cristina; Lopes, Sofia; Ferreira, MargaridaDurante a gravidez, a mulher experiencia mudanças fisiológicas e biomecânicas, podendo resultar em lombalgia e diminuição da atividade física e capacidade funcional. Identificar os parâmetros do exercício na redução da lombalgia e da incapacidade funcional da gestante, sem complicações obstétricas. Pesquisa de estudos publicados entre janeiro de 2013 e 26 de junho de 2018 nas bases de dados e motores de busca PubMed, Science Direct, Google Scholar, CINAHL Complete, MEDLINE Complete, Web of Science, SciELO, Cochrane Library e PEDro para identificar os artigos randomizados controlados que incluíssem os termos: physical exercise AND low back pain AND functional disability AND healthy pregnant AND randomized controlled trial. A análise e seleção dos artigos foi realizada por dois investigadores independentes, obtendo-se um consenso recorrendo a um terceiro investigador cego. Foram identificados 26.012 artigos, tendo sido incluídos cinco artigos que tinham como outcome principal a dor lombar; destes, apenas três apresentaram a análise do outcome secundário de incapacidade funcional. A pontuação média do total de artigos analisados foi de 6,2 em 10 pontos, na escala PEDro. Nestes estudos participaram 817 gestantes, entre os 20 e os 40 anos e com idade gestacional entre a 12ª e a 35ª semanas. O treino aeróbio, o fortalecimento muscular e o relaxamento foram os tipos de exercício mais comuns. Os parâmetros do exercício variaram, entre estudos, na periodicidade (1 a 4 vezes/semana), duração da sessão (30 a 60 minutos/sessão), tempo de intervenção (4 a 12 semanas), com/sem supervisão e em grupo/individual. Após intervenção com exercício físico, a intensidade da dor lombar reduziu em 4/5 estudos e a incapacidade funcional reduziu em 2/3 estudos. Conclusões – A escassez de estudos não permitiu identificar os parâmetros do exercício na redução da lombalgia nem na incapacidade funcional da gestante. Contudo, parece que o exercício físico permite reduzir a lombalgia durante a gestação.
- Physical activity during pregnancy and its effects on neonatal outcomesPublication . This research was deemed necessary with the primary objective of identifying possible associations between adherence to American College of Sports Medicine recommendations on neonatal outcome variables at birth. An observational, longitudinal and prospective study focused on a sample of 70 pregnant women (the power score achieved was estimated to be 0.3). Data was collected in three stages (1st and 2nd trimester of pregnancy and 48 h after delivery). The variables analyzed included the physical activity measurement (with accelerometry), neonatal outcomes at birth and the mother's anxiety, dietary intake, anthropometric measures and socio-demographic and lifestyle. Results: 21.4% women were active according to physical activity recommendations. There were no significant differences in neonatal outcomes at birth when comparing adherence to physical activity recommendations and the overall activity levels of the mothers. There were also no statistically significant differences in the birth weights of babies born to active and inactive mothers. Mother's anxiety level, weight gained and dietary intake were not statistically different according to compliance with physical activity behaviour or recommendations. There is no relationship between adherence to physical activity recommendations on neonatal outcome variables at birth. There were also no significant associations with sociodemographic and lifestyle variables. There is a need to identify attitudes associated with negative physical activity behavior to reduce the incidence of comorbidities associated with sedentary lifestyles during pregnancy.
- Physical activity patterns during pregnancy in a sample of portuguese women: a longitudinal prospective studyPublication . This study had two goals: 1) to analyze PA patterns during pregnancy according to weekly time spent on different types of activity; and 2) to determine women’s perception about health care providers regarding PA advisement during pregnancy. Patients and Methods: A longitudinal prospective study was carried out with a 118-pregnant women cohort. Participants were evaluated during all trimesters. Self-reported questionnaires were used to collect personal and obstetric data. Type, duration and frequency of PA were evaluated using the pregnancy physical activity questionnaire (PPAQ) and intensity levels were calculated. Repeated measure analysis of variance was performed to determine differences between trimesters, and Wilcoxon signed-rank test was performed when appropriate. A decrease in values of self-reported PA (MET.h.wk-1) was found from the first to the second and the first to the third trimester of pregnancy, respectively; total (270.91 vs 220.54 vs 210.35; P < 0.01), light (109.45 vs 95.11vs 92.40; P < 0.01) and moderate intensity (81.99 vs 50.69 vs 62.94; P = 0.002). Time spent on most activities remained fairly stable throughout pregnancy. Women spent most of their weekly time during the entire pregnancy on household and caregiving activities, occupational activities and leisure, except sport activities. Swimming was the most reported organized PA, reaching its highest proportion (12.7%) in the second trimester. Prenatal exercise classes were reported by 39.8% of women during the 3rd trimester. Pregnant women reported that PA was recommended by health professionals: 53.9% in the 1st trimester, 70.4% in the 2nd trimester and 56.8% in the 3rd trimester. Self-reported PA decreased, especially from the first to the second trimester, in total, light and moderate intensity. Women spent most of their weekly time on domestic, occupational and leisure activities, except sport activities. There are some health care providers that do not recommend physical activity during pregnancy.
- Prevalência e impacte da incontinência urinária e stresse. Antes e durante a gravidezPublication . Santos, Paula Clara; Mendonça, Denise; Alves, Odete; Barbosa, AlcindoIntrodução: A Incontinência Urinária (IU) feminina é um importante problema de saúde pública, quer pela sua elevada prevalência, quer pelo elevado impacte físico, psíquico e social na vida da mulher. O objectivo deste estudo foi determinar a prevalência, o impacte da Incontinência Urinária de Stresse (IUS) antes e durante a gravidez em parturientes do distrito de Viana do Castelo e determinar a percentagem de mulheres que procura apoio de um profissional de saúde para o seu problema. Método: Realizou-se um estudo transversal considerando uma amostra representativa do distrito, constituída por 336 mulheres, cujo parto ocorreu no Hospital de Santa Luzia, no período compreendido entre 15 de Janeiro a 29 de Março de 2002. Todas as mulheres foram submetidas a um questionário no pós-parto hospitalar. Resultados: A prevalência da IUS, definida como Alguma vez teve perda de urina durante a realização de um esforço? foi de 5,4% (IC 95%: 3,0-7,8) antes da gravidez e 51,5% (IC 95%: 46,1-56,9) durante a gravidez actual. Os factores associados à ocorrência da IUS antes da gravidez foram a multiparidade (OR=9,96), a presença de diabetes (OR=4,61) e obesidade (OR=4,76), e à IUS durante a gravidez foram a multiparidade (OR=1,66), a diabetes (OR=2,62) e a obstipação (OR=1,73). A grande maioria (88,9%) das mulheres com IUS sente-se incomodada por se sentir húmida, 48,5% sente-se nervosa ou ansiosa e 57,3% tem medo que os outros se apercebam do odor. Durante a gravidez, apenas menos de metade das mulheres que tiveram perdas de urina procurou apoio de um profissional de saúde, apesar de a maioria ter interesse em tratar o problema. Conclusões: A IUS afecta um grande número de mulheres deste distrito antes e durante a gravidez. A IUS tem reflexos em várias dimensões da saúde, sendo o bem-estar físico e emocional os mais afectados, mas apenas uma pequena percentagem de mulheres revela o seu problema de IUS a um profissional de saúde. Perante esta evidência tornase importante que os profissionais de saúde conheçam esta realidade e se preocupem em dar resposta a estes problemas de saúde.

O aumento de peso excessivo pode levar ao desenvolvimento de diversas patologias, presentando relação com maior retenção de peso no pós-parto e risco de obesidade.
