Lower urinary tract symptoms (LUTS) are common in men over 40, compromising quality of life (QV), sexual health, and mental health. The association between LUTS and sexual dysfunction (SD) is recurrent, highlighting the need for culturally adapted and validated instruments for clinical assessment and research. To cross-culturally and linguistically adapt the International Consultation on Incontinence Questionnaire – Male Sexual Matters Associated with Lower Urinary Tract Symptoms Module (ICIQ-MLUTSsex) into European Portuguese and analyze its psychometric properties in men with LUTS. The methodological validation included translation, back-translation, expert review, and cognitive pre-testing (n = 11). The psychometric properties were tested in a cross-sectional sample (n = 84) considering validity and reliability. Content validity was assessed by an expert panel; construct validity was evaluated by exploratory factor analysis (EFA); criterion validity was analyzed by Spearman correlations between the ICIQ-MLUTSsex, International Prostate Symptom Score (IPSS), and International Index of Erectile Function (IIEF-5); and reliability was estimated by Cronbach's alpha coefficient (internal consistency) and intraclass correlation coefficient (ICC) (temporal stability with administration at two time points one week apart). The ICIQ-MLUTSsex showed high internal consistency (α = 0.8). The ICIQ-MLUTSsex showed moderate positive correlation with the International Prostate Symptom Score (IPSS) at M1 and M2 (rho (M1) = rho (M2) = 0.5; p < 0.001), strong negative correlation with the International Index of Erectile Function (IIEF-5) at M1 and M2 (rho (M1) = rho (M2) = -0.7; p < 0.001) and very strong positive correlation between M1 and M2 (rho = 0.98; p < 0.001), demonstrating excellent reliability. The ICIQ-MLUTSsex proved to be valid and reliable for assessing SD in Portuguese men with LUTS.
