- Cross-cultural adaptation and validation of the assessment of chronic illness care (ACIC): Chronic care model — Information for chronic disease managementPublication . Leal, Filipe; Morais, Carminda; Pimenta, RuiDuring the last decade, many countries have been reorient their health systems in order to progressively include integrated management of chronic disease, where diabetes mellitus (DM) is also included. The Chronic Care Model (CCM) has often been the theoretical framework supporting the action accordingly and the Assessment of Chronic Illness Care (ACIC) the most used instrument in measuring their level of implementation. Thus, the present study aims to contribute to improving information systems and clinical decision support in the management of diabetes as a Chronic Disease. Also aimed to make the translation and validation of the ACIC 3.5 and assess what level CCM is applied. The ACIC was applied to 175 doctors and nurses in primary care, belonging to a group of health centers. We defined the level of significance of 5%. In terms of results, it is observed that the ACIC Version 3.5 achieved a Cronbach's alpha of 0.958, indicating a high reliability and a coefficient of Kaiser-Meyer-Olkin of 0.918, demonstrating that our data show excellent suitability for factor analysis. The group of health centers studied only guarantees a basic support to people with diabetes. In terms of applicability of the results of the CCM used in the care provision of the diabetic person and the family, let predict a long journey to make in this area.

- Empowerment and knowledge as determinants for quality of life: a contribution to a better type 2 diabetes self-managementPublication . Ferreira, Pedro L.; Morais, Carminda; Pimenta, Rui; Ribeiro, Inês; Amorim, Isabel; Alves, Sandra MariaThe purpose of this study was to assess how knowledge and empowerment impact the quality of life (QoL) of a person with type 2 diabetes, leading to better communication and disease management. We conducted a descriptive and observational study of individuals with type 2 diabetes. The Diabetes Empowerment Scale-Short Form (DES-SF), Diabetes Knowledge Test (DKT), and EQ-5D-5L were used, in addition to sociodemographic and clinical characteristics. Evaluating the variability in the DES-SF and DKT in relation to the EQ-5D-5L and identifying possible sociodemographic and clinical determinants were conducted using univariate analyses followed by a multiple linear regression model to test whether the factors significantly predicted QoL. A total of 763 individuals were included in the final sample. Patients aged 65 years or older had lower QoL scores, as well as patients who lived alone, had less than 12 years of education, and experienced complications. The insulin-treated group showed higher scores in DKT than the non-insulin-treated group. It was also found that being male, being under 65 years of age, having no complications present, and having higher levels of knowledge and empowerment predicted higher QoL. Our results show that DKT and DES are still determinants of QoL, even after adjusting for sociodemographic and clinical characteristics. Therefore, literacy and empowerment are important for the improvement of the QoL of people with diabetes, by enabling them to manage their health conditions. New clinical practices focused on education, increasing patients’ knowledge, and empowerment may contribute to better health outcomes.
- Ganhos em saúde em utentes internados em unidades de media duração e reabilitaçãoPublication . Almeida, Diogo; Amoedo, Natália; Morais, Carminda; Nunes, Alcina; Pimenta, RuiO envelhecimento da população, o aumento das patologias crónicas e as mudanças de perfil da rede familiar obrigaram a repensar os cuidados de saúde. Criou-se, assim, a Rede Nacional de Cuidados Continuados Integrados, sendo a Unidade de Média Duração e Reabilitação (UMDR) uma das tipologias de cuidados e foco de análise do presente estudo. Pretende-se avaliar os ganhos em saúde obtidos com o internamento em UMDR ao nível do estado cognitivo, do risco de queda e da independência funcional; relacionar o estado cognitivo no momento da admissão com a independência funcional no momento da alta; analisar os ganhos a nível do estado cognitivo, do risco de queda e da independência funcional em função do género. Realizou-se um estudo observacional, analítico e retrospectivo, segundo a metodologia STROBE. Partiu-se da análise documental inerente à base de dados da UMDR, dos doentes internados no período de 1/01/2017 a 31/12/2020, perfazendo 342 indivíduos, dos quais 33 foram excluídos. Recolheram-se dados relativos à caraterização sociodemográfica, ao momento de admissão e ao momento da alta do estado cognitivo, do risco de queda e da independência funcional, avaliados respetivamente através dos seguintes instrumentos, Short Portable Mental Status Questionnaire, Berg Balance Scale e Índice de Barthel. Definiu-se o nível de significância de 5%. A amostra foi constituída por 309 indivíduos, maioritariamente do sexo masculino, 56% (n=173), com média ± Desvio Padrão de idade 73,9±12,2 anos, casados 39,8% (n=123), reformados 83,8% (n=259) e com um diagnóstico principal de acidente vascular cerebral 45,6% (n=141). O estudo evidenciou ganhos em saúde estatisticamente significativos ao nível do estado cognitivo, do risco de queda e da independência funcional (p≤0,001). Verificaram-se diferenças estatisticamente significativas no nível da independência funcional no momento da alta, atendendo ao estado cognitivo inicial (p≤0,001) e, entre géneros, ao nível dos ganhos em saúde de estado cognitivo (p=0,016), risco de queda (p≤0,001) e independência funcional (p≤0,001).
- Health gains with the intervention of rehabilitation nurses in patients with COPDPublication . It is thought that by 2030 COPD will be the third cause of mortality worldwide. The rehabilitation nursing plays a key role in maintaining the functional capacity of the patient with COPD, for its impact on their quality of life. The aim of this study is to evaluate the gains in QOL in patients with COPD who had intervention of rehabilitation nurse, compared to patients who did not have intervention of rehabilitation nurse.
- Incremento no grau de autonomia em pacientes com AVC internados em Unidades de Convalescença da ULSAM-EPEPublication . Esteves, Nury A.; Morais, Carminda; Pimenta, RuiO Acidente Vascular Cerebral (AVC) é a primeira causa de morte em Portugal, afectando sobretudo a população a partir dos 50 anos de idade, sendo responsável pelo internamento de aproximadamente 25.000 doentes por ano. A reabilitação, a readaptação funcional e a reinserção social e familiar é um processo que se impõe, sempre que possível, desenvolvido em contexto de proximidade, designadamente em Unidades de internamento da Rede Nacional de Cuidados Continuados. Pretendeu-se estudar o perfil dos doentes com AVC internados, nas Unidades de Convalescença (UC) da Unidade Local de Saúde do Alto Minho (ULSAM EPE) e analisar o incremento no grau de autonomia para actividades da vida diárias (AVDs).
